Case Manager I (Social Worker)

is seeking a Case Manager for our Medicaid Waiver Dept.
Duties: A Case Manager assesses needs and determines eligibility of prospective clients for waiver services through home visits.
The Case Manager develops care plans, coordinates and monitors services, maintains case files, records, and billing.
Minimum
Qualifications:
ýRegistered Nurse in AL or bachelorýs
degree in social work or related field from an accredited university.
ýProfessional work
experience preferred. Previous work experience with Medicaid Waiver program preferred or any combination of education, training and experience that is commensurate with the requirements of the job.
ýDrug screen, background checks, motor vehicle record.
Starting Salary: $43,
867.20.
